A comprehensive toolkit designed to support Home Care Workers in delivering meaningful activity and engagement for individuals receiving end of life care at home. The resource recognises the importance of supporting emotional, social and spiritual wellbeing alongside physical care, helping providers take a truly person-centred approach to end of life support. Includes practical guidance, positive practice examples and adaptable activity ideas to encourage comfort, connection, dignity and meaningful moments for individuals and those close to them. Developed following consultation with NAPA members and informed by shared experiences and inclusive practice across the sector.
